Study highlights importance of therapy compliance to reduce cardiovascular disease mortality

Patients with rheumatoid arthritis (RA) who discontinue use of statin therapy are at increased risk of death from cardiovascular disease and other causes. According to the findings of a population-based study now available in Arthritis Care & Research, a journal published by Wiley-Blackwell on behalf of the American College of Rheumatology (ACR), RA patients should be advised of the importance of compliance to their statin therapy to reduce cardiovascular disease (CVD) mortality risk.

A report from the World Health Organization (WHO) estimates that RA affects up to one percent of the population in developed countries. Studies have shown that death rates among those with RA are 1.5-fold higher than in the general population, with CVD cited at the leading cause of mortality in this patient group. Statinsdrugs such as atorvastatin (Lipitor) and rosuvastatin (Crestor) that are used to lower cholesterol and manage heart diseaseare a common therapy for RA patients who are at greater risk of heart disease. Previous research reported 38% of RA patients permanently discontinue statin therapy, consequently increasing their heart attack risk by 67%.

"Our study provides evidence of the harmful effects of ceasing statin therapy," said lead author Mary De Vera, Ph.D., with the University of British Columbia School of Population & Public Health and Arthritis Research Centre of Canada. Using data from the British Columbia Ministry of Health records, researchers indentified 37,151 RA patients who received health services between January 1996 and March 2006. Of those with RA there were 4,102 patients who used statins. The team defined statin discontinuation as non-use of the prescribed medication for three months or more, anytime during the course of therapy.

The mean age of the RA group was 67 years, with 60% of the group being women. More than 16,144 person-years of follow-up were recorded for patients using statins, with roughly 45% of statin users discontinuing therapy at least once during the 4-year follow-up period. The authors reported 198 deaths from CVD and 467 deaths overall. Of the CVD deaths, 31% were from heart attacks and 15% from strokes.

Further analysis revealed that statin discontinuation was associated with a 60% increased risk of CVD deaths and 79% for deaths from all causes, which was not moderated by timing of the first statin prescription, age, or gender. "RA patients who discontinue statin therapy are at increased risk of death from cardiovascular disease," concludes Dr. De Vera. "Our study findings emphasize the importance of medication compliance in RA patients who are prescribed statins."

(AP) ? Tens of thousands of people gathered Sunday to attend the highlight of Pope Benedict XVI's visit to this violence-troubled country: an open-air Mass in the shadow of the Christ the King monument, one of the most important symbols of Mexican Catholicism.

With roads closed, pilgrims walked on foot for miles with plastic lawn chairs, water and backpacks. Old women walked with canes. Some Mass-goers wrapped themselves in blankets or beach towel-sized Vatican flags, trekking past vendors selling sunhats, flags, potato chips and bottles of juice.

Hundreds of young priests in white and black cassocks shouted "Christ Lives!" and "Long Live Christ the King!" as they waited to pass through the metal detectors.

Benedict wanted to come to Guanajuato state specifically to see and bless the statue, which Pope John Paul II always wanted to visit but never did, said the Vatican spokesman, the Rev. Federico Lombardi.

The 72-foot (22-meter) bronze monument of Christ with its outstretched arms serves as a potent reminder to Mexicans of the 1926-1929 Roman Catholic uprising against the government and its anti-clerical laws that prohibited public Masses such as the one Benedict will celebrate before an estimated 350,000 people.

Guanajuato state was the site of some of the key battles of the Cristero War, so-called because its protagonists said they were fighting for Christ the King. Tens of thousands of people died before peace was restored. The region remains Mexico's most conservatively Catholic.

The statue "expresses an identity of the Mexican people that contains a whole history in relation to the testimony of faith and those who fought for religious freedom at the time," said Monsignor Victor Rene Rodriguez, secretary general of the Mexican bishops conference.

Benedict will fly over the monument, modeled after Rio de Janeiro's Christ the Redeemer statue, by helicopter on his way to celebrate Mass and will bless it from the air, Lombardi said. After nightfall Sunday he will remotely inaugurate its new lighting system.

On Saturday, Benedict met with President Felipe Calderon in Guanajuato city and later told about 4,000 children massed in the colonial-era city's Peace Plaza that they are each a "gift of God to Mexico and the world."

"The disciple of Jesus does not respond to evil with evil, but is always an instrument of good instead, a herald of pardon, a bearer of happiness, a servant of unity," Benedict said. "I will pray for all of you, so that Mexico may be a place in which everyone can live in serenity and harmony."

He called on the young to be messengers of peace in a country traumatized by the deaths of more than 47,000 people in a drug war that has escalated during a government offensive against cartels.

Eight relatives of victims of violent crime were invited to meet the pope as he left Guanajuato's government palace, Calderon's office said. Lombardi noted it wasn't a sit-down meeting so much as a brief greeting.

While the pope drew a rapturous response from the faithful, his second day in Mexico was not without criticism, particularly concerning the church's treatment of children and sexual abuse.

Victims of Marcial Maciel, the founder of the Legionaries of Christ, launched a book Saturday containing documents from the Vatican archives showing that Holy See officials knew for decades that Maciel was a drug addict who sexually abused his seminarians.

Alberto Athie, former priest and one of three co-authors of "The Desire Not to Know," called on Benedict to publicly recognize the church's responsibility for Maciel's abuse.

"The church won't fall. On the contrary, it will be reconstructed," Athie said.

The 84-year-old pope, who will be going to Cuba on Monday, did not directly address the scandal during his limited remarks Saturday. But Lombardi said his words about the need to protect children from violence referred also to the need to protect them from priestly sexual violence.

Lombardi insisted that neither Benedict nor John Paul covered up for Maciel as the book alleges. Benedict, when he was Cardinal Joseph Ratzinger, headed the office that received the sex abuse complaint from Maciel's victims in 1998; it took the Congregation for the Doctrine of the Faith eight years to sentence Maciel to a lifetime of penance and prayer.

"I have to say that both popes were men, men of truth and transparency," Lombardi said. "I think it's truly unjust to consider Pope Benedict as someone who worked against the truth and transparency."

Benedict has taken up John Paul's drive to reach out to young Catholics, rallying millions of young faithful to join him for World Youth Days, the Catholic youth festivals held once every three years. And he has made the young a focus of his trip to Mexico.

That fits with the Vatican's drive to re-evangelize parts of the world where Catholicism has weakened, trying to rally the next generation to embrace a faith that their parents may have abandoned. While Europe has been Benedict's focus to date, Mexico also has seen its number of Catholics fall.

"The Mexican church feels like it's lost a few generations of Catholics," said Joseph Palacios, a professor of Latin American studies at Georgetown University, citing the battles over liberation theology that drove many left-leaning Catholics away. To get back its numbers, the Mexican church is "moving forward with the new generation," he said.

Tens of thousands, many of them teenagers, on Saturday watched the pope's remarks on big screen televisions set up in a pilgrim camp at the site of the papal Mass planned for Sunday. The atmosphere was like a rock concert as the TVs showed the pope emerging on a balcony in Guanajuato, with campers letting go a roaring chant of "Benedicto, Benedicto."

"We young people are getting closer to the church and to God, instead of getting closer to drugs and violence," said Juan Daniel Pacheco, 18, of Apaseo el Grande in Guanajuato state as he sought shade with his friends at one of the campgrounds that were quickly filling with faithful arriving for Sunday's Mass. "We are young people who will be able to change Mexico."

Of the 43.5 million Mexicans under age 20, 36.2 million are Catholic, or 83.2 percent, just under the national average. The largest group of Mexicans overall are children aged 5 to 9 ? a prime target for Benedict's efforts to rebuild a church that has fallen victim to the same secular trends that have emptied churches across Europe.

At the entrance to Guanajuato, Benedict received the keys to the city and then traveled by popemobile past faithful crowded along the cobblestone streets of the historic colonial-era city.

People packed narrow streets, balconies and rooftops and cheered wildly even as it started to sprinkle. Children and teenagers ran through crowds as Benedict passed to catch another glimpse of him. At one point, someone handed him a baby through an open window of the bulletproof popemobile. He kissed the baby and an aide passed the child back.

A group of 13 indigenous teenage girls traveled for 20 hours cramped in a van from northern Mexico just to see the pope officiate Sunday's Mass. Once in Guanajuato, the Tarahumara got an invitation from the office of Mexico's first lady, Margarita Zavala, to be in the crowd standing right beneath the balcony where Benedict gave his message to Mexico's youth.

The girls, aged 12 to 17, spent three days sewing their traditional dresses for the pope's visit.

Sister Sanjuana Oliva Briones, a Tarahumara Indian who led the group, said the girls were excited and happy with the experience.

"For many it is the first time they have been to a city. Others understand little Spanish," she said.

Nine charged over 14-year-old's gang rape at party

MINNEAPOLIS -- Nine suspected gang members and associates were charged Friday in a sexual attack on a 14-year-old girl who was forced back inside an abandoned St. Paul house and raped after trying to leave a party last November, authorities said.

The girl and her friend were picked up at school by one of the men and lured to a party with suspected members of the TB22, or True Blood 22, street gang, prosecutors said.

"This crime was every parent's nightmare," Ramsey County Attorney John Choi said in a statement. "Crimes like these shock the conscience of our community."


According to a criminal complaint, the girl wanted to leave the Nov. 17 party at the abandoned St. Paul house but was pulled from a car and carried back inside.

"The victim reported grabbing whatever she could, including door frames, to stop them from bringing her into the bedroom," the complaint said. The girl told a nurse that once she was forced onto a mattress on the floor inside the bedroom, a group of men held her down while she struggled and yelled for help.

One of the defendants told police at least two men raped the girl, according to the complaint.

Citing complaints and petitions filed in Ramsey County District Court, the St. Paul Star Tribune?reported the alleged victim heard one?suspect say, "Are you in? I call second."

The girl told the nurse that when she asked the man who brought her to the party, 24-year-old Mang Yang, to help, he said he had to go, according to the complaint. The assault stopped when one of the men yelled "police" and everyone ran, the complaint said.

'Get them drunk'
According to the complaint, Yang told police he knew the others planned to rape the girl and that "is the gang's intent every time they get girls to a party with them to get them drunk and rape them."

One of the teens told police that Yang told him to hold the girl down. But the complaint said Yang told police he went looking for the girl after she was carried from his car and that he heard her screaming and saw teens run out of a room.

Yang said he asked her if anything happened but she was crying and didn't answer, according to the complaint.

Yang is charged along with four other men, ages 18 to 37. Four male minors, ages 15 to 17, also are charged. They each face four counts: aiding and abetting first-degree sexual conduct, conspiracy to commit first-degree sexual conduct, kidnapping someone under age 16, and committing a crime for the benefit of a gang.

All nine defendants are suspected gang members or associates.

Yang and three of the other adults charged made their initial court appearances Friday and were appointed public defenders.

"This is a very serious case involving many co-defendants and we look forward to providing the best defense possible in each case to reach a fair resolution," said Kelley Malone-O'Neill, managing attorney at the Ramsey County Public Defender's office.

Speaking to the Star Tribune, police spokesman?Todd Axtell added: "We are looking for other potential victims to come forward."

?Belfast was a byword for gloom during the Troubles that spanned from the late 1960s to late 1990s, but it is experiencing a new vitality.

Ironically this coincides with the anniversary of the tragedy of the RMS Titanic, which sank 100 years ago on April 15 with a loss of more than 1,500 lives. Belfast is set to commemorate this sad milestone with church services and other events.

The city, which tried to ignore the Titanic until recently because of misplaced guilt at her loss, is now celebrating the shipbuilding genius of the Belfast workers who made her the most advanced vessel in the world at the time. Two recent developments helped the Titanic to become a source of pride: the discovery of the wreck in 1985 by Robert Ballard and the 1997 blockbuster movie, ?Titanic.?

Gradually it became respectable to talk about the Titanic in Belfast, and the city?s shipbuilding history will be celebrated during the Titanic Belfast Festival in April.

The new Titanic Belfast museum will be the world?s largest Titanic visitor attraction.

This befits a resurgent Belfast that was nominated by National Geographic Traveler magazine as one of the world?s top tourist attractions for 2012, and with the steady improvement of political relationships, the city looks set for much happier times.

Oil ETFs Ride Crude Prices Higher On Supply Concerns

Oil exchange traded funds are up for the year with crude prices rising above $110 a barrel at one point in February. Gas prices have surged early this year before the summer driving season, and investors are worried about tensions in the Middle East.

The higher energy prices could stifle the economic recovery, says the S&P, but energy traders may capitalize on the momentum through oil-related exchange traded funds.

"We believe the U.S. could slip into another recession if oil prices reach around $150 per barrel for WTI (about $170 for Brent). And if the disruption in the Persian Gulf gets much worse, prices could go as high as $200, in our view," according to a Standard & Poor's research note.

ETF investors may take a look at the U.S. Oil Fund (USO) or the U.S. Brent Oil Fund (BNO) to gain exposure to either the Western Texas Intermediate crude oil or the North Sea Brent oil prices. Investors in these products need to consider the effect of contango, when later-dated futures contracts are more expensive than near-term contracts, which causes a fund to lose money when rolling futures contracts that are about to mature.

WTI crude was trading around $106-$107 a barrel on Friday.

The higher oil prices translate to lower U.S. economic growth, S&P said.

"We expect that each $10 rise would take about 20 basis points off U.S. GDP growth in each of the first two years of the price hike," the S&P added. "This is a bit less than in the past because cheap natural gas and coal prices now are good substitutes for oil products."

Oil prices, though, have been falling off on slowing Chinese manufacturing data, reports Chris Kahn for the Associated Press. Additionally, Saudi Arabia recently announced that it will increase oil exports to bring down oil prices.

"Strong Chinese growth is the main reason behind rising oil prices the last couple of years," Gene McGillian, a broker and oil analyst at Tradition Energy, said in the article. "As we see slower growth, the thought is that energy demand is going to take a hit."

Additionally, household spending will take a hit as higher energy prices mean higher gas prices at the pump.

"Gasoline, motor fuels and fuel oil account for 5.5% of household spending, and rising. We think a $10 rise in crude oil prices will hike gasoline prices by about 25 cents (8%) and cut consumers' purchasing power by about 0.4%, causing 'staycations' to come back with a vengeance," the analysts said.

U.S. Gasoline Fund (UGA) has gained 18.8% so far this year on the heels of rising oil prices.

DirecTV iPad app adds (some) TV and movie streaming 'Everywhere'

After adding live TV streaming features to the DirecTV iPad app last fall, the company is expanding the feature today by pushing an upgrade that lets viewers watch video (but no live TV, yet) even when they're away from home. Dubbed DirecTV Everywhere, it follows up on the company's promises of more video for customer's mobile devices, offering "selected" video on-demand programming for on the go viewing. The Solid Signal Blog has an early preview of the new features with more screenshots and a video, revealing that available content at the moment consists of premium movie channels (HBO networks, Starz, Sony Movie Channel) and DirecTV's own Audience Network. While we wait for more content, other new features include the What's Hot social module pulling from your Twitter and Facebook friends to see what they're watching, and social checkins with Miso. We haven't gotten any official info from DirecTV on the upgrade beyond the changelog, but for now you can check out iTunes for the details.
